Transition Assistance Program
We understand that changing pharmacies is not always an easy task. Skilled Nursing Pharmacy has created a Transition Team to help coordinate and lead the transition of patients from your current pharmacy provider to Skilled Nursing Pharmacy.
Our Transition Team has years of experience with the process and requirements necessary when it comes to changing providers. It will be our pleasure to guide you efficiently through facilitating a seamless pharmacy transition.
Skilled Nursing Pharmacy offers a full intravenous program customized to the needs of your facility. Our highly skilled IV nurses and pharmacists are available to aid your staff in treating residents with IV therapies.
We offer a wide range of Intravenous Therapy Continuing Education Trainings to keep your staff up to date with the latest techniques and safety practices in our industry.
Diabetic Management Program
Skilled Nursing Pharmacy offers a complete Diabetic Program for monitoring blood glucose for diabetes mellitus residents.
Our dedicated Diabetic Management Specialists conduct Quality Assurance audits on all orders and changes of frequency, monitor the quality control log books monthly for each meter in use, provide audits to the facility administrative staff and work directly with the facility’s central supply personnel for inventory count control to ensure adequate supplies are maintained for all diabetic residents.
Equipment On Loan Program
Specialized equipment is a necessity in our fast paced industry. Skilled Nursing Pharmacy conducts an assessment for your facility to determine the most appropriate and convenient medication delivery, storage, and communication systems. Based on that assessment, Skilled Nursing Pharmacy places equipment in your facility to ensure the proper storage of medications and rapid ordering practices (all equipment remains the property of SNP).
